Constructing a Questionnaire

The objective of this activity is to develop a survey research instrument and to plan for the organization, management, and analysis of the data to be collected.

1. Develop a questionnaire to test a number of hypotheses of interest to you. Your survey must include an introduction and at least sixteen (16) questions, including the following.
   1. At least two must be open ended.
   2. At least five must measure social background characteristics.
   3. At least six must measure attitudes and opinions.
   4. At least two methods must be used to measure opinions.
   5. At least one question must involve more than ten response categories.
   6. At least two questions must be a combination of a filter question and a contingency question.
2. Develop a codebook to accompany your survey instrument. The codebook should include coding categories for your open-ended questions.
3. State five hypotheses that relate variables measured by your questions.
